  • Step 1: Registration  

    Registration is completed via the Genesis Parent Portal under the forms tab and look for the applicable season and this year’s Heritage Middle School Athletics Sign-up. 

    Step 2: Medical Requirements

    Students are required to complete the Preparticipation Physical Evaluation Form ONE TIME per calendar year. It is valid for 365 days.  This form must be completed and only the second page of this packet, the medical eligibility form, is returned to Heritage Middle School nurse. For fall sports, the forms are due by August 15thPlease keep a copy for your records. Please do not email forms.

     

    For every new athletics season during the school year (Fall, Winter and Spring) the student's physician is required to complete and retain the Athletic Health History Form .  Only the third page of this packet, the medical eligibility form, will be returned to Heritage Middle School nurse. The first three pages remain on file at the physician’s office.

    Intramural Sports FAQ

    What are Intramurals?

    Intramurals are recreational sports that allow everyone, of all athletic abilities, to enjoy and have fun while participating in the activities they like.  Students participating in the intramural program will receive instruction before participating in the games/activities.

    Who can join intramural sports?

    Everyone can join! All you need is an updated sports physical and sign up through Genesis. You may ask the school nurse for sports physical details.

    Do I have to sign up or register for intramurals?

    Yes, your parents need to sign you up through Genesis, and you may join intramurals at any time throughout the year.  All you need is an updated sports physical.

    Are intramural sports highly competitive?

    The intramural instructor will have multiple levels of competitiveness based on your ability.

    Can I play more than one intramural sport?

    Yes, you may participate in as many intramural sports as you want.

    Interests or Ideas

    If you have any interests or ideas about intramural sports, we would love to hear them. Intramurals are designed to give all students a chance to participate, have fun, and try new activities in a low-pressure environment. 

    Whether you want to suggest a sport, recommend a tournament format, or share something that would help make the program more enjoyable for everyone, your input is important. Feel free to speak up and let us know what would make intramurals exciting and engaging for you.

    Preparticipation Physical Evaluation Form is required by the State of New Jersey for students in grades 6-12 participating on a school-sponsored interscholastic or intramural athletic team or squad and must be completed prior to a student’s participation as per N.J.A.C. 6A:16 Programs to Support Student Development. 

    Included in the State of New Jersey mandate regarding participation in sports, physicals are valid for one calendar year only. All physicals MUST be documented on the official State of New Jersey physical forms, which are available for download using the link below.  

    Preparticipation Packet 

    A valid athletic physical must be on file with the school nurse prior to the start of each athletic season.

    If the athletic physical has not expired by the first day that practice begins for the selected sport, that physical will take the student athlete throughout the entire season, even if the physical may expire during the season. 

    If the athletic physical expires prior to the first day of practice for the selected sport, a new athletic physical must be completed by your pediatrician or qualified medical professional to participate.   

    We recommend that the parent keep copies of these forms prior to submitting them to the school nurse. 


    IMPORTANT NOTE:  If a student has asthma, an Asthma Treatment Plan must be on file in the student's health records. If you have not already done so, please have an Asthma Treatment Plan form completed and return it along with all other paperwork.  

    Parents are asked to have their own family physician conduct the athletic physical, however; if the family has no personal physician or if the family cannot afford to obtain an athletic physical on their own, they may take advantage of the free athletic physicals given at the school. These physicals are performed during the month of June each year in the Health Office at Livingston High School.

    If your personal physician is administering the physical examination, his/her findings must be recorded on the official Preparticipation Physical Evaluation form. Please be sure to complete all appropriate sections and have a parent/guardian sign the form prior to visiting your physician. 

    The due date for submission of all LPS forms is the same date that the season's physicals are given.

  • SCHEDULES FOR THE WEEK OF MAY 26TH

                                                      

    Track - No practice today.  Final practice will be tomorrow from 3:30-4:30.  Please make sure to bring your jersey tomorrow if you received one.  Uniforms should be clean and placed in a plastic bag with your name on it.  

    If you cannot attend the practice tomorrow, uniforms can be dropped off with Mrs. Mullin in the guidance department. 

    All uniforms need to be returned by Thursday, May 28th.

    Boys Tennis - Coach Desamours will be collecting uniforms tomorrow after school by the tennis courts.

    If you cannot attend, uniforms can be dropped off with Mrs. Mullin in the guidance department.

    Baseball - Coach Murray will be collecting uniforms on Thursday, May 28th, after school in the main gym. 

    Uniforms should be clean and placed in a plastic bag with your name on it.  

    If you cannot attend, uniforms can be dropped off with Mrs. Mullin in the guidance department. 

    All uniforms need to be returned by Thursday, May 28th.

    Softball - Coach Newman will be having an end-of-the-season party tomorrow at 3:15 in the cafeteria.

    Please bring your uniform with you or drop it off at Coach Newman's classroom beforehand. 

    Uniforms should be clean and placed in a plastic bag with your name on it. 

    All uniforms need to be returned by Thursday, May 28th.
